The state of Indiana offers three ways to request a crash report. These three methods are online requests, phone requests through a subcontractor, and in-person requests at your local Indiana State Police department, or in some cases, your local police department. How to request a crash report online in Indiana (recommended)

When requesting your crash report online in Indiana, you’ll need to pay a $12 fee for your request. For your online request, you will need to be able to provide the following information:

  • The jurisdiction of the area in which your crash took place
  • The location of the incident
  • The date of the incident
  • Your name and information
  • The report number of the incident (this is obtained from the law enforcement officer that reports to the scene of your crash)

After doing so, you’ll be taken to a page where you’ll be able to input all of the relevant information regarding your incident into an online crash report request.

How to request a crash report in person in Indiana

The alternative to an online crash report request in Indiana is an in-person request. When requesting your crash report in person, the office cited by the state government of Indiana’s website to go to is located at:

  • Any other documents or information advised by the police department
