How do I request a collision report by mail in Hawaii?
When sending your request for your MVC (motor vehicle collision) report, you’ll be required to include a decent chunk of information relating to yourself and the incident, according to the Honolulu Police Department, regardless of which island you live on. Some things you will need to know/have for your request are:
- The name(s) and address(es) of the complainant/victim/involved party
- Police report number
- The date and time of occurrence
- The request letter MUST BE SIGNED AND NOTARIZED to confirm identification
- A birth or death certificate from the involved party may be required if you are requesting as next of kin
- A self-addressed stamped envelope with enough postage and a large enough envelope to mail return your Police report (the Honolulu Police Department does not fax or Email)
- Payment is required in the form of cash, check (payable to the City and County of Honolulu), or Cashier’s check
The cost of a copy of a report is $0.50 for the first page and $0.25 for each additional page. It is also advisable that if you request your MVC report via mail, that you call the police department responsible for filing your report and verify how much payment is required so you know exactly what to include in your request. The Hilo Police Department’s records division is responsible for all of the island’s police reports. To request a collision report from the Hilo Police Department, contact them at (808) 961-2233 to find out if there is any additional information that your request would require along with:

- Police report number
- The date and time of occurrence
- Appropriate Payment

The Maui Police Department is in charge of filing all police reports for Maui, Molokai, and Lanai. To request a collision report from the Maui Police Department, contact them at (808) 244-6355 to find out if there is any additional information that your request would require along with:

- The date and time of occurrence

Hawaii Accident Reports - FAQ
In Hawaii, collision reports are filed by the police department that is responsible for your island. There isn’t a centralized office that you can contact to request reports for incidents on all islands. To obtain your collision report, you’ll have to contact the police department responsible for your island in particular. Note that in Hawaii, you cannot have your collision report faxed to you, these reports can only be obtained in person and through mail. On top of this, expect to pay between 50 cents and $2.00 when requesting your collision report. While this is on the cheaper side compared to most other states, it’s important to remember that there is a payment required when requesting these reports. Lastly, Hawaii doesn’t have a designated request form for motor vehicle collision reports, so you will need to write or type your requests yourself when mailing them.
On Kauai, reports that regard a case with pending criminal charges cannot be requested. On Maui, Molokai, and Lanai, reports regarding pending investigations cannot be requested.
For most of Hawaii, you will need to provide the following information to find your accident report:
- The name(s) and address(es) of the complainant/victim/involved party
- Police report number
- The date and time of occurrence
If a car accident results in injury, death, or property damage valued at $3,000 or more, law requires you to call the police immediately. In the case that a driver is unable to call the police, someone else must do so.
