How do I find my accident report in Colorado?
Driver Record Requestor Release and Affidavit of Intended Use (Form DR2489)
To obtain a copy of an accident report you were involved in you need to complete form DR2489 (Requestor Release and Affidavit of Intended Use) and pay the required fee of $9 (plus $1 optional if you want it to be a certified record). When requesting this report, keep in mind that you ended to allow 90 days for receipt of the accident report by law enforcement agency and to be data entered into the driver license system. The department maintains accident reports for seven years.
Basic State of Colorado Crash Report
There is also a basic crash report that doesn’t include witness statements, digital media, or other crash investigation elements. The basic crash report can be requested online and there is a $5 fee to obtain one.
To complete the online request form, you will need the following information:
- Case number
- Date/Location of Crash
- Involved Party Name(s)
Alternatively, you can fill out a request form CSP 402 and send it to Colorado State Patrol at [email protected] or you may mail it in, along with payment to the Colorado State Patrol Central Records Unit 700 Kipling St., Lakewood Colorado. You can also fax the request and payment form to 303-239-4347. The fee is $5 for up to 10 pages and the price goes up for 11 or more pages as outlined on the form.

Colorado Accident Reports - FAQ
For a basic accident report that can be ordered online, you need:
- Case number
- Date/location of crash
- Involved party name(s)
For the Driver Record Requestor Release and Affidavit of Intended Use (Form DR2489), you will need to fill out the form. The information that needs to be provided is:
- Name of driver
- License number
- Date of birth
- Police department
- Report or care number
- VIN
- Date of accident
- Accident location
- City/county
For the basic accident report, you simply need to agree/affirm that the requested information will not be used for solicitation or business for monetary or pecuniary gain and acknowledge that such a violation is a misdemeanor and is punishable by a find and/or imprisonment.
For the DR 2489, you must check one of the following:
- Be a government agency, including any court of law or law enforcement agency performing its functions for an approved purpose under DPPA
- Be an agency charged with driver/motor vehicle safety or theft including: MV Product alterations, recalls, advisories, MV performance monitoring, MV parts/dealers, MV market research or surveys, removal of non-owner records from original owner records of MV manufacturers.
- a business that will use the information to verify the accuracy of information submitted by individuals for the purposes of preventing fraud, pursuing legal remedies against or recovering a debt or security interest.
- In connection with a civil, criminal, administrative or arbitral proceeding in any court or before a self-regulatory body, including investigation, execution of judgment, or pursuant to a court order. (Attach form DR 2478 Driver Record Statement of Fact)
- In research activities (the information may not be published, redisclosed, or used to contact the parties)
- For use by a licensed private investigator, licensed private investigative agency, or licensed security service (Provide license number)
- By an insurer or insurance support agency in connection with claims, investigations, anti-fraud activities, rating or underwriting.
- By an employer/agent or insurer of a Commercial Driver's License holder. (CDLIS or CMV record)
- Other as permissible pursuant to 24-72-204 (7) C.R.S. (DR 2478 Driver Record Statement of Fact required)

If there was not a police officer present at the scene, you can file an accident report online here. The form allows you to report accidents that occurred less than 60 days ago which was not investigated by law enforcement.
Keep in mind that if an accident report is filed online by an individual, the report doesn’t get investigated by a law enforcement agency. They are kept for record purposes only and you should make sure to provide the other driver with your: name, phone number, insurance information.
