On 12/18/2023 I was contacted by Clerk Wolfe in reference to this report being request by the State of Ohio due to a fatality being involved. I contacted Det. Pentz who provided me a copy of the coroner's report that was not in the case file indicating that the cause of death was listed as drowning. I updated the crash report to a fatal and requested Clerk Wolfe send it to the State of Ohio. At the time of this submission; I do not have any of the toxicology reports.Unit 1 drove into the driveway access area near 3639 Stoneway Point. Unit 1 drove off the left side of the roadway; down an embankment; before driving into a pond. The only damage observed was to the front end of the vehicle where it struck an unknown object or the pond bottom.   Unit # 1 was an Lexus ES350                     Based on the initial autopsy data Unit # 1's driver sustained a contusion on his leg that would not have caused his death. Due to NHSTA MMUCC 5th addition manual Fatal Injury definition this crash does not arise to the required level. When the final autopsy report is obtained this report will be updated if necessary.
