Additional Narrative

Unit #2 was traveling westbound on I475W when Unit #1 drove eastbound from the Promedica Pkwy entrance ramp onto I475W and struck Unit #2 head on. The driver to Unit #1 was transported by TFD to the Toledo Hospital and was pronounced deceased at 0518hrs by Dr. Cotterman. Supplemental report written 11-07-22. Correction to the occurrence date on page 1 to the original accident report. The accident occurred on 11-05-22 and not on 11-04-22
