This accident, involving 3 vehicles occurred on Sunday, December 12th 2021 at approximately 6:47 pm. The driver of silver Ram (2006) swerving to avoid. Injuries were reported. An officer from Columbus Police Department was present at the scene and recorded the information contained in this report.
Unit 1 was traveling along US 33 EB towards I-270. Unit 2 was traveling in the local lane of US 33 WB towards SR 104. Unit 3 was traveling in the fast lane of US 33 WB; towards SR 104. The listed witness stated that a gold SUV was changing lanes on US 33 EB; when Unit 1 quickly came upon it. The witness stated that Unit 1 swerved left to avoid hitting the gold SUV and lost control. The witness stated that Unit 1 went over the median and into the lanes of US 33 WB where it was struck by both Unit 2 and Unit 3. The driver of Unit 2 stated that when Unit 1 crossed the median; her vehicle was struck by Unit 1 on the rear diver's side. The driver of Unit 2 stated that Unit 1 then bounced off of Unit 2 and was stuck head on by Unit 3. Unit 2 suffered functional damage and no injuries were reported. Unit 3 suffered disablling damage. The driver of Unit 3 was transported to Grant Hospital by CFD Medic 23 in stable condition. The front seat passenger of Unit 3 was transported to Mount Carmel East Hospital in stable condition by CFD Medic 4. The rear passenger of Unit 3 was transported to Grant Hospital in stable condition by Madison Township Medic 183. The driver of Unit 1 was pronounced deceased by CFD Medic 23 at 6:55 PM. CPD Accident Investigation detectives responded to the scene. The driver of Unit 1 had to be extracted from the vehicle by mechanical means and was transported to
