UNIT #1 was traveling S/B on STATE ROUTE 57 south of STATE ROUTE 301 in the right lane. UNIT #2 was traveling S/B on STATE ROUTE 57 in the right lane approaching UNIT #1. UNIT #3 was traveling S/B on STATE ROUTE 57 in the right lane approaching UNIT #2. UNIT #4; a semi-truck with flatbed trailer; was traveling N/B on STATE ROUTE 57 in the left lane approaching STATE ROUTE 301. UNIT #4 lost control and swerved left into the grass median; crossing the median and entering the S/B lanes of STATE ROUTE 57. UNIT #4s front end then struck UNIT #1s front end head on. UNIT #1 was pushed W/B from the impact into the west berm of STATE ROUTE 57. UNIT #1 came to rest facing E/B on the west berm. The rear trailer of UNIT #4 jackknifed and rotated counter clockwise causing the trailer to strike the left sides of UNIT #2 and UNIT #3. UNIT #2 came to rest in a ditch on the west side of SR 57 and UNIT #3 came to rest on the west berm of SR 57. Both units were facing E/B. The driver and passenger of UNIT #1 were deceased at the scene. The driver of UNIT #2 was lifeflighted from UH-Elyria to UH-Cleveland for head and body trauma. The driver of UNIT #3 complained of chest pains and was taken to UH-Elyria. The driver of UNIT #4 was conscious and sitting by UNIT #4. He stated; "I don't know what happened" to Officer Cooley. He was taken to UH-Ely
