This accident, involving 2 vehicles occurred on Tuesday, August 24th 2021 at approximately 9:41 am. The driver of black ran stop sign. Injuries were reported. An officer from Findlay Police Department was present at the scene and recorded the information contained in this report.

Unit 1 (bicycle) was traveling westbound on Center St. approaching the Tiffin Ave. intersection. Unit 2 was traveling eastbound on Tiffin Ave. in the left lane. Unit 1 failed to stop at the stop sign on Center St. at Tiffin Ave. Unit 1 rode northbound across Tiffin Ave. and into the path of Unit 2. Unit 2 struck Unit 1 on the left side. Unit 1 was ejected from the bicycle. Driver of Unit 1 sustained fatal injuries as a result of the crash. Unit 2 reported no injuries. Unit 1 sustained disabling damage and was placed into Police Property for safe keeping. Unit 2 sustained minor damages and was driven from the scene. On 8-24-2021 at approximately 0941 hours; I was dispatched to an injury crash involving a bicycle on Tiffin Ave. in the 400 block. Dispatch advised the bicyclist was bleeding from the head. Upon arrival I found a white 2019 International box truck; bearing Ohio registration PKK5100 and a black/tan Schwinn bicycle to be involved. The box truck is owned by Brandon and Sons LLC out of Dayton Ohio and was being operated by Jacob Tackett. The operator of the bicycle was identified as Michael C. Lafferty. The box truck was stopped in the middle of the left eastbound lane just east of the railroad tracks. It was facing eastbound. The bicycle was on its side. The front tire was pointing east and the rear tire was pointing west. I noted significant damage to the
