Supplemented Information 11/28/2020: urine test results entered for driver of Unit #2 Anthony Naylor. Supplemented Information 9/15/2020: additional notes added to OH2 narrative and updated information for Unit #1. Unit #1 was traveling westbound on SR 5. Unit #2 was traveling eastbound on SR 5. Unit #2 traveled left of center causing Unit #1 to swerve left. Unit #2 then traveled back into the eastbound lane striking Unit #1. After impact Unit #1 traveled off the south side of the roadway striking a ditch.
