Unit 1 was driving west on SR 665 just west of Lambert Rd. Unit 1 failed to maintain reasonable control and slid off the left side of the road into a ditch. Unit 1 then slid down across the ditch; and struck a tree next to the ditch; which caused it to go airborne; and strike a driveway drain culvert. Unit 1 then rolled over onto its top and slid across the driveway. The driver of Unit 1 was ejected from the motor vehicle; and landed in the middle part of the roadway. Pleasant Township Medic #231 pronounced the driver deceased at 2040hrs.
